If you wish to utilize an insurance benefit, we would be more than happy to take your insurance information over the phone. We will call you before the day of your appointment and let you know about your benefits and copay. Also, we will need to make a copy of your insurance card on your first visit.

Every client needs to complete the 1st four forms. Then complete the information form associated with the client's age (adult, adolescent or child). If the client is under 18, please complete the Consent to Treat a Minor form.
